Resources are produced in resource production sites and used by heavy industry sites to produce supplies. But how are the resources moved from site A to site B?

I have skimmed the manual, but been unable to find anything yet. If this movement require the use of strategic movement points, I REALLY need to start thinking of what location I evacuate my Soviet factories to.

Also, if this is the case, where does all the off-map produced resources enter the map?

Similar questions can be asked on how supplies are moved from the Heavy industry sites to all the production facilities that consume supplies.

Furthermore, browsing old threads there has been some discussion on factory damage being dependant on evacuation distance. Am I correct in assuming that this is no longer the case?

You don't have to care about moving resources and supplies to production sites. The game will automatically do it for you without any cost.
Edit and yes you're right the damage to factories evacuated is indipendent from distance
