Does terrain have any affect on combat?

I see that it affects stacking, so rough terrain is worse for defence as you can't stack as many units in it?? Or do I misunderstand?

How do rivers affect anything in the game?

we surmise that terriegn affects combat, I am sure rI saw that here or in the manual, river ?? no idea

There's a terrain display on combat results that includes a combat modifier - which is effectively an entrenchment bonus for the defender I think.  Frank did say somethign about rivers here to I seem to recall...

You loose 1 readiness by crossing a river which adversely affects combat.

Rivers do affect readiness as they're crossed, increasing it by one. Also, if they are in a combat hex the same impulse they moved across the river they have a slightly higher chance of taking casualties.

Also, if more than 50% of the forces on one side crossed a river into that combat hex the other side's trench benefit for the battle is increased for that impulse.
