I would like to be able to have the option to sub players, particularly QB's from the individual team level instead of a setting in the constants file.

Too many QB's are playing in the game for one team and I don't think that is realistic.

Some type of setting in the team playbook that says if a player reaches a certain fatigue level then he comes out. When he is rested to a certain level he comes back in.

Im pretty sure this has been discussed before. Dont remember the outcome, but it was quite a while ago.

I dont mind too much when a backup QB comes in for a play, but.....I have seen the backup come in when it's 3rd and 7 with the game on the line. I dont imagine the AI is taking that into account, it's just subbing for a fatigued player. But Dungy wouldnt pull Peyton on a crucial play if he's tired. Peyton wouldnt let him know he's tired if it was a crucial play.

As I recall, the only position not pulled for fatigue on CPU controlled teams is the QB.

QB's do leave the game for injuries.

This is an issue that has been discussed recently.
