Only half? Pfffft... I've gone in with an entire CAG sitting on the deck, and the crews listening to a VD lecture.

Playing too often with threat tolerance, direct ways and remain on station or reaction radius buttons... got involved several times in stupid fights or undesired withdrawals or having dozen of subs pursuing a few convoys instead of reaching their patrol zones just by forgoting about coming back to the appropriate status/radius... Not big mistakes just annoying. To avoid checking this up all the time I decided to apply systematic doctrines when defining missions - no order change later.

A classical one. Having once a big invasion convoy loading troops/supply then puting it in stand by a few days to coordinate with the incoming naval sweeping operation to repulse various TFs - thus secretly unloading - then sending it empty for a 6 day dangerous but crucial travel... then sending it back when realizing but of course the crucial target hex was lost before reloading the desappointed soldiers...

I should add that sending a 12DD TF to raid my fast BB Surf TF was a big experimental mistake of an opponent. Indeed allied DD's had always been no match vs jap BB/CA's in my multiple surface combats - at long/medium range during day combat BBs are now real monsters against small ships exploding one by one whereas during night CA LR torp are even more terrible - and the beta patch seems to allow catching DD escorts+convoys more often than before - no more quasi-automatic escape so by insisting... my "moonlight night convoy hunting jap surface TFs" achieved bloody surprises and hurted repeatedly my opponents this week. A lot of marines swimming back...

Important ships in front line bases should never be placed under pierside repair for minor damage. If you do, they can't get away when the KB comes calling.

Not changing the combat mode of an amphibious task force that having successfully invaded Guadalcanal was then loaded up with supplies and oil and confused with a second amphib task force and sent to amphibiously assault another Jap held island..... It gloriously stormed ashore with full bombardment support....... and unloaded half the oil and supplies to the grateful Jap garrison ........

I had PoW sitting at Surabaya undergoing pierside repair (too big for the shipyard) when a mini-KB showed up and began sinking all my transport TF's. I sweated bullets for several turns until they left, having never bombed Surabaya's port.

I love these logistics "Tales of Folly" even more than the combat "oopsies". They immediately convey a visual of some poor Lieutenant (who drew the short straw) quivering before the Admiral's desk, as the be-starred one looks up over his glasses and loudly exclaims, "You people did WHAT??!!??"

Back in UV I had several task forces that I wanted to travel together to meet up and move as a group.  One of them was an Amphib group.  I chose a spot on the map and had all the task forces move to that spot with remain on station.  The problem was I picked a dot hex and when the amphib showed up, they assaulted the small coral head.  And because there was no faciliteis at this port, it took a LONG time for them to climb back aboard their ships.
