I have been playing v4.0 for a couple days now. (Yes, I'm working on getting v4.5 but having some download problems.) The last time I played the tutorial, around turn 10, some of my units, it seemed, would no longer accept new formation objectives. All the units indicated they were in contact, I had enough orders and could set the objective hexes, but the stance of the units remained the same, it never changed. Am I doing something wrong here, or is there a known problem in this area? Also, is there a file out there which details the fixes/changes of the various versions or from v4.0 to v4.5? Or do I need to download the upgrades to see this info?

The problem with accepting new objectives may be a function of the communication distance btween units. Your '0' command unit may have 4 orders but he may not have sufficient to get all of the units to receive them. Move your command unit next to the unit that is not responding and next turn, you should notice that the offending unit has set the objective- same as the command unit.

Actually each unit in the formation was adjacent to each other. (4 M10s abreast). Originally C0's and C3's stance changed (in the second and fourth position left to right). I finally had to right click and select change stance for the other two units individually (at the cost of additional orders). The interesting thing I hadn't noticed prior to my last post, was that even though the stance never changed originally (it was still defend) the coordinates had changed to the coordinates of the new objective hex. So I had two units which had a stance of advance to XX,XX and two with defend XX,XX. (The coordinates were not the same as the original defend coordinates.) Strange...
