What is up with pirates accepting missions then failing to actually do anything?

I've also got a failed mission from 40+ years ago in my pirate mission log, and it's not going anyway - anyone know how to clear that?

EDIT: Also, is the pirate AI pretty bad? It seems like they could be doing so much more to be a true threat than they really are. There are probably 25 pirate factions in my game, and only one of them has more than like 3 or 4 ships. Maybe the incredibly powerful faction just took all the other ones out? Either way they have like 25-30 frigates and destroyers surrounding my planet - fortunately, I have a defense payout with them for cheap. Shouldn't they be trying to take over my system? Mine it, and build pirate bases and things? They had the corruption on my planet up to 60%+ at one point, and then it miraculously dropped to 3% on a reload. They have several times canceled our treaty just when I started venturing from my planet and destroyed all my ships and bases, so they have essentially locked me onto my planet for 52 years in game, which is really cool, but I just keep waiting for them to actually assert dominance over my planet itself.
