Is there a way to set the threshold at which ships will request to be refueled? I don't want notifications for my ships when they are still 60% full of fuel.

Would setting a home base for my fleets avoid this better? I prefer not to do that, because I do everything manual so they don't need to know their "home base" (if they have one), but I'm just trying to think what is the mechanic that sends the notification, and how to eliminate it on ships that (by almost every measure) really don't need refueling.

I don't believe that setting a home base would have any impact on this. As far as I am aware, setting a home base does two things: it sets the 'defend' target for Defensive Posture, and it gives the fleet a point to which it will return when idle if automated.

The only setting that I can think of off the top of my head that might affect when you get refueling notifications is the fleet refueling percentage under [esc] -> Options -> Empire Options, but I wouldn't count on that having an effect. You can also disable the notification that ships require refueling in [esc] -> Options -> Empire Options -> Notifications, though that is perhaps not exactly what you're looking for.

I'm not sure. My observations lead me to believe that it is not a simple "when less than X% fuel remaining" or "when less than X fuel remaining" trigger, so my next guess would be that the trigger incorporates an estimate of fuel required to reach the nearest refueling point in some manner, perhaps "when [current fuel] - [estimated fuel to get to refueling point] < [X% of fuel capacity]" or "when [current fuel] - [estimate] < [X fuel]."

This is however not something that I've tested at all.

This is part of the supposedly unchangeable automation that drove me so crazy. Foolishly, I wrestled with it and fumed at it before realizing that I must work around it (and hey, I made peace with it so it stopped bugging me, heh). Here's what I have learned: ships with automation on will refuel much sooner than those without automation, so when this matters, be sure to have it off. Also, it's most important to arrange designs/fleets with similar fuel capacities so you don't have your lead hulk pulling everyone else off to refuel and leaving an important base/planet vulnerable--nothing worse than a giant carrier being low on fuel when all the brawlers are near full and ready to attack. I've gone so far as to plan my strategy around where the defensible fuel sources are. Knowing this can be an issue helps me minimize its impact on my grand plan.
