In order to maintain the Burma road, are all 4 locations required, Rangoon and Mandalay, Lashio and Kunming? Or is it at least one of the pair are required?

According to the documentation you need to hold all four locations.

I haven't even seen the option for Burma road. When Nanking falls it has not been asking for the Burma road option.

Burma has to join the Allies to trigger the script. I am beginning to think an early Allied diplomatic blitz in that area might be helpful.
