March/April 1943 interphase Italy left the Axis without any notice (I don't know who they surrendered to but all their territory was listed as Axis control so presumably the surrendered to Germany). Italian morale was 20 due to loss of African colonies, half the navy and miscellaneous casualties. Germans held Rome and 4 other hexes within 15 hexes of Rome. Allies held the Straits of Messina and the one hex on the tip toe of Italy.

The Strategy Guide, page 24 under German Allies says Italy may surrender to Germany if the allies land in Italy (the allies held that tip toe hex and all of Sicily for at least six months previous) and the Germans have 3 units within 15 hexes of Rome.

The general rule for surrender (5.11, page 88) is a major power will surrender if their morale is zero, even if they hold their capital.

To me "may surrender" seems like Germany must accept the surrender, same as with France. However, in this case Germany had neither a chance to reject the surrender offer nor adjust its units to address the Italian units being vaporized instantly.

I'm new so maybe everybody understood this rule, but this rule seems incredibly arbitrary and capricious in treating France one way and Italy another.

Because it's not the same story. In 1940 France asked for an armistice. It's not the case for Italy. In 1943 Mussolini was arrested by Italian authorities, Italy change side. German army reacted promptly, take control of Rome and half of Italy, sink an Italian BB and desarmed all Italian military units. In several cases Italian soldiers were executed, in Greek Islands by exemple. In Corsica Italian occupation army helped French resistance to take control of the island.

Then in 1940 Germany could say yes or no to the French demand. 1943 there wasn't any Italian question.

I believe that you are confused concerning the requirements for Italy to surrender to Germany. It isn't a decision event. It is called Operation Axis. This is what is in the strategy guide:

Operation Axis
If there are at least 3 German units within 15 hexes of Rome when Italy’s National Morale falls below 20%,then Italy may surrender to Germany and have her remaining territory taken over by the Germans.
It is therefore recommended to deploy at least 3 German units in Italy when the Allies are close to invading. It will also be best to place German or Axis Minor units in Tirana in Albania before Italy's National Morale falls this low, as otherwise Albanian Partisans will liberate the country when Italy surrenders.

I would suggest that if Italian national morale gets anywhere close to 20%, be sure to have 3 German units within 15 hexes of Rome in Italy, garrison Tirana, the capitol in Albania and move German or German minor allied units into any currently occupied hexes (like in the Soviet Union) that you want to remain Axis. All Italian occupied hexes outside of Italy will become Allied controlled. Any Italian controlled anti-partisan hexes will usually generate an Allied partisan when Italy surrenders.

If it were me, I would have all Italian ground and air units back in Italy before Italy surrenders.
