Using 1.67BETA, I make changes to my lineup then go to Manage Game and the LineUp Card on screen before Game Start has all been changed. I then have to make changes again to LineUp Card so I can us the players I want. I thought maybe the AI Manager was resetting the Lineup because of Player Usages was set. Next game I Cleared all Player Usages and it still happens.
I don't think this is how it worked before 1.67B.

Maybe "Auto Juggle Lineups" is turned on? Check Mgr/GM Tendencies screen for your team.

I looked but there does not seem to be a way to turn it off. I thought clearing the Player Usage would do that for me. The choices the the AI Manager was making didn't seem to follow the settings I had set anyway. It was putting an injured player back in the line up as well as players more tired than the setting called for.

Does this happen to you? Or do you have a different way of playing the game.

I've come to find that the AI managers are much more "hard-as*ed" that I'd ever consider being, starting tired players. Way back then, sitting out because of a minor hurt, or just being tired, was considered unthinkable.

As hard as Shaun might try to improve the AI, he can't give it a conscience. The flipside is that human managers (I'm one of them) sometimes project human attributes on these virtual players, so I guess there's a tradeoff involved. I know better, but the more you play, once in awhile your subconscious humanity overtakes you. That's when you start making decisions on a hunch. It happens.

It is possible that the reason I never noticed this before is that I also recently changed the settings on the Mgr/GM Tendencies screen. Maybe it isn't that 1.67B changed the behaviour. I will have to expirement, I'll just try to put the settings back to where they might have been, if I can remember what they were:)
