I have a few ships with assault pods that are permanently stuck at 2/3 or the like. Perhaps it's related to the issue I've seen now and then with the pods after they are fired getting stuck in mid-space if the target is destroyed. Combining that with the behavior that ships appear to not fire if their current projectile is still en route, even when they seemingly should be able to due to the weapon fire rate might explain the issue.

I had this happen again in my current game, and what I saw happen was I attempted to capture an enemy ship, I landed one assault pod, the enemy ship promptly blew up for reasons unknown, and a 2nd assault pod then froze in mid-flight. With other weapons, the projectile immediately goes away when the target ship is destroyed. If it's not going away with assault pods, combine that with what is apparently a game mechanic that doesn't let your weapons fire if the current weapon's projectile still exists (even if you should be able to fire based on the weapon's firing rate)... well that might explain why the assault pod isn't reloading.
