Attached is a scenario with CG 70 Ticonderoga cruiser defending against a salvo of DF-21 missiles. For ilustration I've halved the amount of SM-3 on magazines, everything else is stock. 1) First launch is with SM-3 missiles. During separation of DF-21 warheads SM-3 lose lock, go autonomous and harmlessly pass about 40km below warheads. I believe it is caused by warhead separation changing parameters of engagement beyond SM-3 missile envelope. I do not know if this is a bug or everything works as intended. During tests I've also found a workaround: based on trials, warheads separate at 55-57% of distance from launcher to target. When detecting a ballistic missile, set all available weapons to hold fire, measure (or guess if not possible) the distance between launcher and target and determine if your radar is tracking a missile, or a warhead. In the first case wait until separation (your radar will lose lock and one sweep later acquires warhead), in the second case you can launch immediately. 2) Second launch (of SM-2 Blk IV) never happens, because cruiser's radar illuminator can't lock on warheads and provide guidance, even shortly before impact. For some reason AN/SPG-62 is incapable of illuminating DF-21 RV. I am sure this is not working as intended.
