In the manual I read something about a campaign game. Yet I find no campaign scenario's. What am I missing?

It's just a longer scenario which has some phases where it's "night" so has a forced vision limit. None shipped with the game, but a few are being worked on and you're of course free to make your own with the Editor.

I have the 'skeleton' of one on paper but haven't started inputting it into TotH yet because my Otto Carius scenarios keep getting in the way.

No, from what I understand they are basically just massive, longer time frame (turn) scenarios.
