There are those with more expertise than I about Japan but as far as Melanesia is concerned, pretty much all of the Solomons and PNG are Malaria Endemic areas. Vanuatu and New Caledonia are quite Malaria clear. I highly recommend Vanuatu as it is easy to get around, not obscenely expensive, has a lot of interesting culture, and a fair amount of history. Plus the only accessible Cargo Cult in the World (The John Frum cult on Tanna). And some awesome Volcanoes. I'm planning to be there in a few weeks for the 3rd time and I'll try to get some pictures and post them.
