Ok got this message 2 days in a row.

Troops and Equipment from 802nd EAB lost during loading of TF 146

This is during the loading of an Amphib TF at Annette Island.

Any help?

Loading from small ports with no possibility of docking a ship will sometimes lose you some devices/squads. Accidents are ready to happen when ropes swing

I also get this at times when unloading at small ports, even got it once at Auckland when it was I think size 7. I figure the desingers must have put in something to account for faulty or insufficient unloading equipment

jamesjohns you might have been unloading but not docked. Heavy equipment to unload or load needs to be docked or might just be dropped into the drink.

At small ports, larger pieces of equipment are transferred to shore or ship by lighters. A lot of crane work and rigging, or even block and tackle work is involved.
In addition to equipment failures, the experience of the ship's crew and units ashore might play a part in the calculations. Poor experience does not go well with rigging and hoisting/swinging a large, awkward and heavy item. I think the abstraction of a small port into odds of failure in offloading items with large load cost probably includes these sorts of considerations.

You can reduce the risk of loss on those "no/small port" landings if you have a command unit with them, (to organize things.)
IIRC it will only happen on amphib landings, or places where you meet an enemy present.
