Can you pick back up by air? Or are the troops stuck?

Stuck, or they can walk out.
You’ll need a level 1 airfield for transport aircraft to land and pick them up.

Thanks so if it is an island the only way back out is naval transport I guess?

Patrol planes can pick them up from islands

Yes, so can APDs or other fast transport capable ships.

Select a Troop Transport mission, pick the destination (where the pick-up is needed), select "Pick up Troops" mission, then select the unit to be picked up. If only one unit is there it will automatically be selected for pick-up. You may need several days to pick-up the entire unit. Watch the Patrol Aircraft pilot and airplane fatigue levels and stand down the mission if severe weather/thunderstorms are at either base.

Or put 20-40% on rest, depending on distance.
