I'm not sure this has been addressed, but players don't line up in the correct positions that you define in the formation editor.
I'll explain. You position a DE lined up over the Offensive tackle, but when you create a play with that formation then practice it the DE is lined up over the guard instead and the rest of the line is out of whack as well. The yardage markers in the play editor don't correspond with the actual field during play.

Hmmm.. thats interesting. You might want to post this Toast in the support forum. If distances and yards are wrong in the PDS compared to actual game play, that could lead to serious problems with Play design.

I think someone said it is because there are two different fields for the outdoor game. The default for the defense play design and the offense play design or different - or something like that. This causes the misalignment. You can change the fields in the play editor.
