A problem that I have always been curious about is when certain formations go into reorganization mode without ever being in combat, or under any sort of major stress. How does this happen?

Their force may have a shock event handicapping them. At shock levels of less than 100, a formation has a possibility of going into reorg, even with no combat or losses to it.
