There could be many valid reasons for that - no fuel/ammo, too high odds, missed rolls, badly placed staging bases, etc..

If you launch manual mission you need to select staging base, otherwise it will select the one with biggest amount of planes. This is WAD and was described in the past.

You may attach a save, but your report so far doesn't indicate automatic problem.
