I am designing a South American scenario where Venezuela and Cuba faces Brazil, Guyana, USA and UK for the Essequibo and Northern Brazilian Amazonian region. During the test play I´m facing Nuclear attack unexpectedly. It always happens in the first few turns. Only some USA and UK forces equipment have Nuclear capability (some ships). Sometimes it occurs in the ocean (hex 0,0), sometimes in land hex owned by USA/UK troops. There are no events that could be triggered to those attacks happen. What could be causing those attacks? In TOAW IV, what are the situations where nuclear attacks are initiated inadvertently? If you prefer I can send you a copy of the scenario, so you can help me debugging it. Thanks in advance.

Wild guess here, perhaps the Air Staff Assistant is trigger happy.

Bur you can post your scene in this thread maybe someone can figure it out. I think you have to zip the file to post it.

You could start the game with Nucs disabled and then enable them with an event when you figure out what turn you'll start using them on the map. I've seen Elmer launch nuclear attacks before.

Disabled is the default condition (see attached shot).

They are enabled by event. Once enabled, I don't think there is a way to go back to disabled.
